Banks Online Complaint Form
Before you begin, please review instructions on how to file a complaint with the Division of Banks.
To file a bank complaint, please provide:
- Your name, mailing address, and phone number (with area code).
- The name and location of the bank.
- A simple explanation of the problem, with a list of events in chronological order, making sure to include names and dates. Be specific and as brief as possible.
CAUTION: All information you provide will be disclosed to the bank listed below. If your complaint involves a bank that is not regulated by the Division of Banks, then the matter will be forwarded to the appropriate governmental regulatory agency.
