Washington Exchange Facilitators Survey
Engrossed Second Substitute House Bill 1078 (Chapter 70, Laws of 2009) (PDF)* requires exchange facilitators operating in Washington to provide certain information to the Department of Financial Institutions (DFI) about their activities during the calendar year 2009.
DFI has prepared an online survey as a way to simplify the collection the required information.

Before taking the survey, make sure you have collected all the information you need. -
Take The Online Survey
Fill out the survey online. Once completed, the survey will automatically be sent to DFI.
Information Must Be Submitted By December 31, 2009
Regardless of how you submit the information, it must be received by DFI by the close of business on December 31, 2009. This requirement applies to all persons and entities serving as exchange facilitators in Washington, regardless of size, type of business organization, and physical location of the facilitator. Failure to provide the necessary information may result in civil or criminal penalties.
Use of this survey to submit the required information is voluntary and you may choose to provide the information in an alternative format as long as it is received by December 31, 2009.
