How To Re-Apply For A Loan Originator License
There are several steps to reapplying for a Loan Originator license. Follow the steps below in order. If steps are performed out of order, you risk your application being rejected.

Complete the Loan Originator License Application Manually
Open and print the Loan Originator license application (PDF). -
Mail the following items to DFI:
- A manually completed Loan Originator license application (PDF)*.
- $125 check made payable to the Washington State Treasurer

A completed paper copy of the MU4 authorization form (PDF)*. Have it notarized then mail it to DFI. Submit a separate MU4 form for each Mortgage Broker you work with.
Note: The MU4 form does not replace your online application. You must apply online as well. - A completed fingerprint card. Fingerprint cards can be obtained from DFI or at your local law enforcement office. Review the fingerprint card instructions (PDF)* on how to complete the cards.
- A copy of the Promissor certificate showing that you passed Loan Originator licensing test.
- A copy of your two Continuing Education courses.
Note: The address from which you conduct loan origination activity must be a licensed Mortgage Broker location.

If you completed the steps above, your complete Loan Originator application should include the following 6 items:
- A manually completed Loan Originator application
- Check for $125, made payable to Washington State Treasurer
- Complete MU4 form
- Fingerprint card
- Copy of Promissor certificate showing you passed the test
- Copy of your two Continuing Education certificates for 2007
Once DFI has received and approved your completed license application including fingerprint card, Promissor certificate, $125 check, manually completed application, two Continuing Education certificates and notarized MU4 form, DFI will issue your license.
